Turmeric lowers heart attack risk post surgery
London, Apr 17: Turmeric may help lower heart attack risk in people post bypass surgery, thanks to curcumin, the yellow pigment present in the spice which has antioxidant and anti-inflammatory properties. Bypass surgery improves blood supply to the heart muscle. However, during the surgery that is usually performed with the heart stopped, the organ can be damaged by prolonged lack of blood flow, increasing the patient's risk of heart attack. The new findings suggest that curcumins may reduce such risks when added to traditional drug treatments. The results need to be confirmed through further research, said Wanwarang Wongcharoen from Chiang Mai University in Thailand, who led the study, the American Journal of Cardiology reported. Researchers at the Thai university studied 121 patients who had non-emergency bypass surgery between 2009 and 2011. Half the patients were given one-gram curcumin capsules to take four times a day, starting three days before their surgery and continuing for five days afterwards. The other half took the same number of drug-free placebo capsules. The researchers found that during their post-bypass hospital stays, 13 percent of the patients who'd been taking curcumins had a heart attack, compared to 30 percent in the placebo group. After accounting for any initial pre-surgery differences, Wongcharoen and his colleagues calculated that people on curcumins had a 65 percent lower chance of heart attack. "It's very, very encouraging," said Bharat Aggarwal of the study. Aggarwal works with the MD Anderson Cancer Center in Houston, Texas, which studies the use of curcumins in cancer therapy, according to the Daily Mail. According to researchers, it is likely that the antioxidant and anti-inflammatory properties of curcumin may have helped limit heart damage in the patients.

A new research into the disease fighting properties of Garlic has turned up fresh evidence about its health benefits. Researchers from the University of Adelaide have found that an extract from the humble clove of garlic can help treat high blood pressure. The finding was reported in the latest edition of the scientific journal Maturitas. However, the researchers caution that according to their studies they did not observe similar health benefits from garlic that was consumed raw or cooked in meals. The benefits were only observed in an extract from aged garlic. The studies were carried out over a period of twelve weeks and involved fifty test subjects.

The good old ginger, the mother nature cure for several illness, is being discovered just like garlic. Ginger, according to ayurveda literature is heat causing or tamasic food like garlic, and onion. In fact, studies measuring body temperature and metabolic rate have confirmed the traditional belief that ginger generates ushma or heat in the body. Adrak or ginger is a perennial plant. The rhizome (the underground stem) is used. Traditional Chinese medicine has recommended ginger for over 2,500 years. It is used for abdominal bloating, coughing, vomiting, diarrhea, and rheumatism. Ginger is commonly used in the Ayurvedic and Tibbia systems of medicine for the treatment of inflammatory joint diseases, such as arthritis. The dried rhizome of ginger contains approximately 1-4% volatile oils. These are the medically active constituents of ginger; as well, they are responsible for ginger's characteristic odour and taste. The aromatic principles include zingiberene and bisabolene, while the pungent principles are known as gingerols and shogaols. The pungent constituents are credited with the anti-nausea and anti-vomiting effects of ginger. Ginger is a classic tonic for the digestive tract. (Soak one teaspoon of freshly grated ginger in a cup of boiling water. Keep it for 10 minutes and drink warm). Classified as an aromatic bitter, it stimulates digestion. It also keeps the intestinal muscles toned. This action eases the transport of substances through the digestive tract, lessening irritation to the intestinal walls. Had a bad day? Want to freshen up? Make a ginger drink. Sprinkle 1/4 teaspoon of ground ginger into a cup of hot water and add 2 teaspoons of honey to make a pleasant drink. For cold and cough ginger is affective too. Take dry ginger, grate them, wram them it up in ghee, let it cool add gur and eat. You will feel the difference. Ginger may protect the stomach from the damaging effect of alcohol and nonsteroidal anti-inflammatory drugs (such as ibuprofen) and may help prevent ulcers. Anti-nausea/anti-vomiting actions: Research is inconclusive as to how ginger acts to alleviate nausea. Ginger may act directly on the gastrointestinal system or it may affect the part of the central nervous system that causes nausea. It may be that ginger exerts a dual effect in reducing nausea and vomiting. Interestingly, some studies have found that when ginger paste was applied on the abdomen of women with breech baies, it could correct the foetal position to a normal one in 75 per cent (as compared to a spontaneous conversion rate of 54 per cent). Ginger also supports a healthy cardiovascular system. Like garlic, ginger makes blood platelets less sticky and less likely to aggregate, although not all human research has confirmed this. This action reduces a major risk factor for atherosclerosis. Besides, it is supportive in conditions namely chemotherapy support, menstruation, painful (dysmenorrhea),migraine headaches morning sickness, motion sickness, nausea and vomiting following surgery, rheumatoid arthritis etc. Nothing conclusive can be said about the curative or supportive aspects of ginger for it has yet to generate as much curiosity as garlic has created in the medical fraternity. Most people take 2-4 grams of the dried rhizome powder two to three times per day or a tincture of 1.5-3 ml three times daily. For treatment of nausea, people try single doses of approximately 250 mg every two to three hours, for a total of 1 gram per day. For prevention of motion sickness, many people start taking ginger tablets, capsules, or liquid herbal extract two days before the planned trip. Given to 80 naval cadets at sea, the ginger juice was very effective. Are there any side effects or interactions? Side effects of ginger are rare when used as recommended. However, some people may be sensitive to the taste or may experience heartburn. Persons with a history of gallstones should consult a nutritionally oriented doctor before using ginger. Short-term use of ginger for nausea and vomiting during pregnancy appears to pose no safety problems; however, long-term use during pregnancy is not recommen

Dr R. VatsyatanAn essential element of the cell chemistry, cholesterol is a fat (lipid) which is vital for the proper functioning of the body. While building the outer cell membrane, it performs multiple physiological functions like helping to make bile and certain important hormones besides metabolizing some of the vitamins and insulating the nerve fibres. Produced by the liver, cholesterol is carried in the blood by molecules called lipoprotiens, a compound containing both fat and protein.When we talk about cholesterol, we are drawn to its three main types — the LDL (low density lipoprotein), the HDL (high density lipoprotein) and the triglycerides. The first one is often referred to as bad form of cholesterol, the excess of which can increase the risk of arterial diseases. The HDL, or its good form as it is called, does the opposite of the LDL and takes the cholesterol away from the cells and back to the liver where it is broken down and expelled from the body as waste. The triglycerides form the blood fat and originate either from fats in the food or are made in the body from other energy sources such as carbohydrates.Thousands of years ago ayurveda described the ‘medo dhatu’ or the fat tissues among the seven types of group of cells which constitute our body. It says that the fat tissue in itself is not bad and is actually essential for the body to function properly. A balanced digestive process is necessary for the healthy formation of the body tissues, including the fats. A bad functioning ‘agni’ or the wrong metabolism supported by dietary and lifestyle indiscipline can disturb the quality and quantity of the ‘medo dhatu’ or can mar the formation of healthy fat or good cholesterol.The formation of high cholesterol or the excessive ‘medo dhatu’ can result due to a number of factors which include high intake of dietary fat, saturated oils, bakery products, eggs, red meat and many other such things. Lack of exercise and adopting sedentary lifestyle, smoking and regular use of alcohol accompanied by fatty snacking are known reasons of cholesterol aberrations. Many times genetic trends and individual idiosyncrasy also play an equally important role to gain bad lipids even from natural foods.According to ayurvedic beliefs, if wrong lifestyle is the reason of high cholesterol a person can generally get his lipids back to normal by adopting certain precautions. After checking the cardiovascular status one should do regular and reasonable amount of exercise. Eating wholegrain cereals, vegetables, fruits with pulp and roughage not only has numerous other health benefits but act as good fillers also. Unsaturated fats, red meat, fried snacks and regular outdoor eating, alcohol and smoking should be reduced to the minimum.Of the numerous prescriptions, the simple Triphala powder is the foremost natural remedy to correct any metabolic disorder, including the cholesterol uprising. Another herb called guggul or Commiphora wightii is the chief ingredient of many of the classic preparations and has a proven anti-lipid action. According to a report by the World Health Oraganisation in 2009 "More than 270 million people are susceptible of falling victim to diseases linked to unhealthy lifestyles in the next few years. Most of these people are thought to come from China, India, Pakistan and Indonesia." Did this ring the alarm bell? For people, who by now have brought in important lifestyle changes, future might become better and for those who believe they are blessed with immunity for good, welcome to the 'party'. For a lot of people who fall in the latter category, cure is as important as prevention. "People get aware about the hazards of unhealthy lifestyles at a very late age, and by that time the body system has already suffered," says Dr. Anil Grover, cardiologist, PGI. Lifestyle diseases like diabetes, hypertension and obesity are not very difficult to handle even after taking precautions. The cause of lifestyle diseases boils down to two factors, physical inactivity and unhealthy eating habits. They remain the same even after one suffers from them. "In case of diabetes or high blood pressure, one need not leave the normal diet. What is important is, eating food in the right quantity and of a desired quality. The patient should stress more on fruits and vegetables," says Dr Parvinder Chawla, attending consultant, internal medicine. Also it doesn't take much to change the physical activity part. "It is as simple as a doing a brisk walk for an hour for five days a week, or going in for strenuous work out in the gym," suggests Dr Grover. Madhu Sharma, dietician, PGI finds it very usual when a patient suffering from diabetes or obesity discusses his or her lifestyle, "As expected, it is faulty most of the times. And we require is to bring in discipline in daily routine." The most common excuses she gets to hear from people is that they skip their meals due to high-pressure jobs and then end up gorging on junk food. Others say that they sleep late and for less time. They have no time for aerobics, yoga or a simple walk. As a suggestion, those suffering from hypertension and diabetes should strictly avoid food rich in saturated fats, sodium and processed food. It's vital to assure that the food is rich in fibre content. Casual approach of the patients can prove to be really hazardous for their health. As Dr Chawla says, "A patient should not blindly follow a medication suggested to him by a doctor after a certain time gap. Regular medical check ups are strongly recommended to assess if the dose of medicine needs to be altered." If prevention is better than cure, cure isn't that bad an option too!

Green tea contains healthful substances that can penetrate eye tissues, raising the possibility that the tea may protect against glaucoma and other eye diseases, say researchers. Scientists have confirmed the benefits of green tea in ACS's bi-weekly Journal of Agricultural and Food Chemistry.Chi Pui Pang and colleagues point out that so-called green tea "catechins" have been among a number of antioxidants thought capable of protecting the eye. Those include vitamin C, vitamin E, lutein, and zeaxanthin. Until now, however, nobody knew if the catechins in green tea actually passed from the stomach and gastrointestinal tract into the tissues of the eye.Pang and his colleagues resolved that uncertainty in experiments with laboratory rats that drank green tea. Analysis of eye tissues showed beyond a doubt that eye structures absorbed significant amounts of individual catechins. The retina, for example, absorbed the highest levels of gallocatechin, while the aqueous humor tended to absorb epigallocatechin. The effects of green tea catechins in reducing harmful oxidative stress in the eye lasted for up to 20 hours."Our results indicate that green tea consumption could benefit the eye against oxidative stress," the report concludes. — ANI

Acute pancreatic — a clinical dilemma Dr J.D. Wig

The pancreas in an elegant organ as long as it is not disturbed, quietly going about the job of producing insulin as well as enzyme that pass into the upper gastrointestinal tract and help breakdown fats and proteins.Pancreatitis is an inflammation of the pancreas. The most common causes are excessive alcohol use and gallstones. These two are responsible for nearly 80 per cent of all hospitalisation for acute pancreatitis. Passage of gallstone or excessive alcohol use cause injury to the pancreas and the gland leaks these enzymes into the surrounding tissue. In a mild case (80 per cent), this causes inflammation and pain. When severe (20 per cent to 30 per cent), this causes tissue destruction on a big scale, resulting in the destruction of the glands (necrosis). The pancreas begins digesting itself and the cycle of inflammation begins, including swelling and loss of function. Digestion of blood vessels results in bleeding.The disease comes suddenly. Pain is a major symptom. This is usually quite intense and steady, located in the upper abdomen and often felt all the way through to the patients back. The patient’s breathing becomes quite shallow because deep breathing causes more pain. Relief of pain by sitting and bending forward is characteristic. Shock may appear in severely ill patients.Patients become extremely cold, and blood pressure drops dangerously low. Major organs of the body are deprived of blood and oxygen resulting in dysfunction or failure of the lungs, heart, kidney, liver and gastrointestinal tract. Patients may have changes in their mental status — may even become comatose (pancreatic encephalopathy). If a patient develops the failure of three or more organs, there is hardly any survivor. Sometimes these patients develop abdominal distension and an increase in intra-abdominal pressure which is a frequent cause of multiple organ failure.In severe cases, pancreas gets swollen and suffers bacterial and fungal infections. The CT scan is the best diagnostic modality which shows necrosis of the gland and local complications. CT acts as a road-map and guides the surgeon for subsequent surgical intervention.The initial management in severe cases is supportive in an intensive care unit. Patients need careful monitoring for organ failure and support. Pain is controlled with a variety of medications and the patient is not allowed to eat for a few days; oxgan in administered by a mask. Large volumes of intravenous fluids are needed. Calcium need to be given as these patients may have a low calcium level. The lung is the commonest system to fail, requiring mechanical ventilation (support of breathing with ventilator).Surgeons are frequently involved with the care of these critically ill patients with tissue destruction — the dead tissue needs to be removed as this not only makes thepatient’s sick but it may also get infected. The surgeon needs to remove this dead infected tissue. One need to search and remove the dead and necrotic tissue with care.Attempts are made to leave viable pancreas and to avoid injury to blood vessels and adjacent structures (stomach, spleen, intestine). This operation of removing dead and necrotic tissue (pancreatic necrosectomy) is a challenging operation. Utmost care needs to be taken when removing the necrotic tissue. Repeat operations may be required to remove the dead tissue. When patients require repeat operations, the risk of complications and damage to adjacent structures increases.These patients are in for prolonged hospitalisation, requiring every resource, and some do not survive. These critically ill patients burn up their calorie reserve and need nutrition via a feeding tube inserted into the proximal small intestine at the time of surgery.Hospital stays are long, complications frequent. Patience on the part of patient, the family and the surgeon is required to get the majority of these patients back on their feet. Those who survive lead a normal life.Alcohol is essentially the only preventable cause of pancreatitis. Avoiding heavy alcohol use will help prevent pancreatitis. Anyone who already has one episode of acute pancreatitis should stop drinking to prevent the conditions from coming back. Gallstones must be treated in patients with gall stone pancreatitis as gall stone pancreatitis recurs in 30 to 50 per cent after an initial attack.Acute pancreatitis that is not related to alcohol use cannot be prevented.The writer, a former Professor and Head, Department of Surgery, PGI, is associated with Fortis Hospital, Mohali.

Washington: Espresso, French roast and other kinds of dark-roasted coffee is not harmful for the tummy as was previously believed because these roasts contain a substance that tells the stomach to reduce the production of acid, according to a study. People who resist from enjoying that much-desired morning cup of coffee because of fear of stomach irritation can also take a sigh of relief because scientists have discovered the culprits behind that heartburn and stomach pain in every cup.
The research may lead to a new generation of stomach-friendly brews with the rich taste and aroma of regular coffee. One of the researchers said the processes used to produce stomach-friendly coffee also could reduce the amount of healthful substances in the coffee, including some that scientists have linked to benefits such as protection against diabetes and heart disease. — ANI

Kidney stones strike more than a million Americans every year, sometimes causing enough pain to bring them literally to their knees.
Along with medication to discourage the formation of kidney stones, sufferers are often encouraged to make dietary changes, among them drinking more citrus juices. Citrate in the fruit reduces the formation of calcium oxalate stones (the most common type) and lowers urine acidity, much like the kidney stone medication potassium citrate.
But not all juices have the same effect. Lemonade or diluted lemon juice is the usual recommendation for people with calcium stones. But a study financed by the National Institutes of Health in 2006 compared lemonade with orange juice in patients with calcium stones and found that three cups of orange juice a day—along with other standard dietary changes for kidney stone patients—did a better job of raising citrate levels and decreasing urine acidity than lemonade or distilled water.
Then there are cranberry and apple juices, which, according to studies, are good for some stones and bad for others. They raise the recurrence risk of calcium stones, but help prevent a far less common subset of kidney stones called brushite. Grapefruit juice, in contrast, raises the risk across the board. One large study in The Annals of Internal Medicine found that a daily cup of grapefruit juice raised the risk of stone formation as much as 44 per cent.
The bottom line
Some fruit juices protect against kidney stones; others raise the risk of recurrence.
